What is Intermediate Care?

The Intermediate Care Team (CSH Rapid Response team) are here to help your transition from acute hospital care back to your own home. A longer stay in hospital may not medically appropriate and the team may suggest alternatives such as care at home or in the community. Their purpose is to make sure you remain independent to live your life and reduced the chance of readmission to hospital.

There are times, however, that this may not be possible. As we age, everyday tasks may become harder and after an accident or illness, it will be harder to readjust as before. The team will assess your needs along with occupational therapists, the clinical teams who treat you, your GP, and a social worker if necessary.

To avoid future injury, the team may be involved to make suggested adjustments to your home to make tasks easy for you to perform. They may also suggest that a care home or a residential rehabilitation unit would be more appropriate depending on your circumstances including your health and social situation. The team also work closely with our patient falls nurses and our End of Life care team.

 

Our Joint Community Hub at St Peter's

The 'community hub’ has opened at St Peter’s Hospital to help make sure patients are discharged from hospital as soon as they need to be, to the most appropriate place and with the right care plans in place.

It is a joint venture between Ashford and St Peter’s, Surrey County Council, Surrey Community Health and the British Red Cross, the unit focusses on helping patients who are staying in the hospital’s short stay Medical Admissions Unit and on those who can be safely and quickly discharged from A&E.
